Transitional Care

Change can be hard and uncomfortable, especially with changes pertaining to health and age. We at Aspen Care understand that these periods of change can create ample of stress on patients and their families. That is why transitional care is a top priority and can have the utmost importance.

“Transitional care refers to the coordination and continuity of health care during a movement from one healthcare setting to either another, or to home. Care transitions can be between health care practitioners and settings as a person’s condition and care needs change during the course of a chronic or acute illness.”

If you are a concerned family member, or friend, you can request an assessment through Alberta Health Services, or by phoning (403) 943-1920. Once a phone call is made, an appointed Transitional Coordinator will provide a complete comprehensive client assessment to determine health care needs for continuing care services. The Transitional Coordinator will:

  • provide information and referrals for Continuing Care Services and/or AHS living options
  • discuss and develop the transition plans with clients and their families including identified goals
  • waitlist clients for appropriate Continuing Care Living Options or alternate levels of care
  • assist the client and family through the transition to another health setting or supported discharge home with Home Care
  • assist with connecting clients and families with other health care teams or community programs as well as recommend non- health related community services and support

The client’s assessment will determine what course of action and type of facility is required for a healthy and comfortable transition. At Aspen Care, we recommend that the family or the client’s advocate remain on top of the transition, and aware of every detail, to ensure the client is in the appropriate continuing care program. Change is never easy, however, the client should eventually feel at ease and happy in their new care facility. Should the client feel uncomfortable or uneasy after a trial period, it could eventually affect their health condition. Therefore, it is extremely important to weigh all continuing care options and conduct plenty of research before placing your loved one into a new care program.

Private care programs are an alternative to public care referrals made by the Transition Coordinators. Aspen Care is a private boutique health care provider that offers transitional services and continuing care services to clients.
